The South West Self-Management Program offers programs for health service providers to assist with the integration and implementation of self-management best practices. The Choices and Changes, Treating Patients with C.A.R.E and Health Literacy workshops provide opportunities to learn and practice self-management support skills and communication techniques that ultimately enhance client-centered care.

Open workshops are facilitated across the South West region and are available to any health care provider. Pre-registration is required. There is no cost to attend a session, but space is limited.
