Helping People Help Themselves
People living with chronic conditions spend about five percent of their time with health professionals. The rest of the time, they manage their own health. That’s why self-management is so important – to people with chronic conditions, to caregivers, to healthcare providers, and to the health system as a whole.
The South West Local Health Integration Network and the South West Community Care Access Centre have partnered to:
Offer the Living a Healthy Life with Chronic Conditions Workshop Series for individuals and their caregivers who are living with chronic disease. These workshops provide information and teach practical skills that are needed to confidently manage their disease.
Support health care providers with delivering client centred care by providing tools and skills training sessions in communication techniques.
