Having the confidence to manage all aspects of your life including daily activities and your medical needs – we call these skills “self-management skills”.
This free and effective 6 week self-management program developed and researched at Stanford University is now offered around the world!
You should attend this workshop if
• you are someone with a chronic health condition (high blood pressure, arthritis, heart disease, stroke, Parkinson’s disease, asthma/COPD, diabetes, obesity etc.);
• you are interested in learning how to better manage your chronic condition;
• you are caring for someone living with a chronic condition.
Participants will receive a copy of the book “Living a Healthy Life with Chronic Conditions”.
This program is offered in partnership by the South West CCAC and the South West LHIN.
